We are seeking a Senior Pharmacy Care Coordinator. You will be focused on interpreting and supporting continuous quality improvement initiatives You will support HEDIS, CMS STARS, and other quality performance metrics.
Full remote position – Working hours: M-F PST or MST anywhere between 7:00am-6:00pm
You’ll enjoy the flexibility to telecommute* from state of AZ, UT, ID, WA, CO, NV, CA, OR and NM as you take on some tough challenges..
Primary Responsibilities:
- Provider group liaison to improve Part D Measures including medication adherence, statin use
- Provide education and formal presentations on Part D measures to provider group staff, including member educational initiatives promoting formulary alternatives and strategies to improve STAR/HEDIS measures
- Understand pharmacy related quality reports and identify areas for intervention and improvement (e.g. medication claim reversals, medication adherence, etc.)
- Assist with preparing educational materials and coordinating initiatives (e.g. provider statin letters, medication adherence prior year non-compliant lists)
- Maintain and educate on patient and provider resources (e.g. financial assistance, educational materials, measure details, etc.)
- Coordinate with outpatient pharmacies to ensure members receive prescriptions timely and assist with prescription refill requests.
- Conduct telephonic patient outreach to educate on medication adherence opportunities.
- Other duties as assigned to meet departmental objectives

Required Qualifications:
- High School Diploma/GED (or higher)
- 3+ years of Pharmacy or Quality related experience
- Intermediate level of proficiency with Microsoft Office applications including Word, Excel, and Outlook
Preferred Qualifications:
- Pharmacy Technician or MA license in good standing, in any state
- Advanced Education (i.e. Associate’s or Bachelor’s degree)
- CPhT certification
- Experience in managed care pharmacy (Pharmacy Benefits Manager, Medicare Advantage)
- Experience using EMR systems
- Knowledge of Medicare part D
- Ability to recognize potential drug discrepancies and barriers to adherence
- Ability to identify and analyze drug data trends
- Knowledge of HIPAA
Soft skills:
- Demonstrate accuracy, timeliness, organization and attention to detail
- Strong customer service and communication skills
- Understanding SDOH (Social Determinates of Health) Factors
