Under general supervision from a licensed mental health therapist, the Behavioral Health (BH) Case Manager is responsible for coordination of care for therapy and psychiatry patients. The Case Manager performs patient/provider matching for transfers of care; serves as a bridge between medical providers and behavioral health providers to implement treatment plans; and works with patients to optimize adherence to their treatment plan.

Primary Responsibilities:

  • Assist in Care Coordination work with medical teams, BH clinicians, psychiatry prescribers in care planning using a care plan template and care pathway protocols. Assists in the creation of care plans and documentation/entry into Healthy Planet once available
  • Assist patients in finding substance abuse treatment in the community. Follows up with patient to promote follow through
  • Assist patients in finding/utilizing community and government resources. May assist patient in completing various applications for services
  • Follow suicide/homicide watch protocol
  • Communicate with family to further the effectiveness of treatment program as allowed by law
  • Provide information needed to hospital for admission. Find beds when patient need to be admitted to a psychiatric or chemical dependency unit
  • Provide warm hand offs to Regional Support Networks (RSN) for patients that qualify and need wrap around care
  • Assist providers in finding resources for patients that don’t quality for RSN and need higher level of support
  • Coordinate with providers and support staff to obtain records from hospitals or outside The Everett Clinic (TEC) psychiatrists
  • Assist in arranging medication injections for psychiatry patients. (Case Managers do not administer injections)
  • Perform chart audits on standard work issues as needed
  • Use motivational interviewing techniques to encourage patient follow through and involvement in their treatment plans
  • Connect patients to resources to obtaining payment plans or applying for financial assistance medical bills
  • Attend required meetings and participates in committees as requested
  • Participate in professional development activities
  • Flexible schedule to meet demand for departmental business needs; may involve some evening hours
  • Review transfer of care requests and assign patients to new prescriber or therapist. Communicates with patient regarding change in providers
  • Coordinates use of emergency slots for Behavioral Health and Psychiatry providers, prioritizing post hospitalization patient needs
  • Oversees coordination of care when providers depart from TEC
  • Recognize when a caller/patient is in need of crisis intervention and arrange for telephone triage with the on-call clinician or arrange same day appointments with a licensed BH clinician

Required Qualifications:

  • Knowledge of mental health conditions
  • Knowledge of basic computer skills and ability to learn and apply new technologies and skills; proficiency using EPIC and MS Office suite
  • Excellent written and oral communication skills
  • Ability to balance multiple priorities; thrive in an environment where urgent matters can cause frequent interruptions
  • Ability to read, speak, understand and write proficiently in English
  • Ability to work independently and under supervision

Preferred Qualifications:

  • Associate level licensure with Washington State for counseling
  • 1+ year of experience as a Case Manager or related position; mental health, crisis intervention, or call center experience

Washington Residents Only: The hourly range for this role is $28.03 to $54.95 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
