Job Summary
The Division of Hospital Medicine program is seeking to hire a physician assistant (PA) and/or nurse practitioner (NP) to join our Hospital Care at Home team. Hospital Care at Home (HCAH) is an innovative care delivery model that offers eligible patients the opportunity to have their inpatient-level hospital care needs met from the safety and comfort of their own homes, enhancing both patient experience and clinical outcomes.
The Division of Hospital Medicine is a rapidly growing, dynamic and evolving group within the Department of Internal Medicine. Clinicians in this role care for patients with a wide range of illnesses and system-based problems, ranging from hematological malignancies to cardiovascular, gastrointestinal, pulmonary, and multi-system disease. In Hospital Care at Home, advanced practice providers (APPs) work in close collaboration with attending physicians to assist with identifying clinically appropriate patients, completing history and physicals, evaluating, and managing patients who are admitted or need admission to HCAH, and formulating diagnostic and treatment plans. The role also includes admitting, triage and cross-coverage responsibilities, consultative services along with procedural and other specialty duties as described below.
The Hospital Medicine APP role is based on collaborative model of care teams made up of physician and advanced practice providers on services including (but not limited to): Medical Short Stay Unit, Medicine Faculty Hospitalists, Medicine Procedure Service, Medicine Hematology co-managed service, Hospital Care at Home and internal medicine resident ward services. APPs in the Division of Hospital Medicine are expected to not only engage in clinical activities and responsibilities but also engage in education and continuous quality improvement activities.

Responsibilities*
Basic Functions:
This APP is responsible for the clinical screening and admission of eligible patients to enter our Hospital Care at Home (HCAH) program from University of Michigan Hospital. The HCAH program provides inpatient level care for many conditions in the patient's home, reducing complications after discharge, readmissions, and improving access to inpatient level medical care. They may also participate in-home visits and perform virtual/video visits in the virtual hospital.
The APP will review patients in the Emergency Department (ED) and inpatient units for HCAH program eligibility. Once potential candidates are identified, the APP will collaborate with hospitalist and ED staff to begin admission process into HCAH.
HCAH Acquisition APP Roles and Responsibilities:
- Conduct review of potential patients via the electronic medical record and identify candidates for transfer to HCAH
- Perform comprehensive in-person assessments and physical exams as well as virtual assessments
- Collaborate with supervising physician to develop and implement a plan of care.
- Enter admission orders
- Order and interpret laboratory and diagnostic studies
- Obtain specialty consultation where needed
- Complete clinical documentation in accordance with institutional standards
- Manage incoming referrals and consults to the HCAH service
- Educates eligible patients/families on the Hospital Care at Home model
- Obtains consents for HCAH admission
- Work closely with HCAH team to understand intake capacity to maximize daily patient intake and addresses obstacles to patient intake
- Communicate and build collegial relationships with clinical staff and multidisciplinary colleagues in brick & mortar referral sites (e.g., Emergency Department, Med-Surg floors, Primary Care)
- Perform hand-offs to Virtual Hospitalists, RNs, and other care team members
- Engage in continuous process improvement and program growth efforts
- Completes comprehensive assessments to identify the physical, psychosocial, behavioral, and environmental needs of the patients to determine if patients qualify for HCAH
- Teaches family members and other care givers symptom and medication management and preventative care.
- May participate in the education of health care professionals and students through mentoring and precepting
- May provide in-services, seminars, and presentations .about HCAH
- Communicates effectively and professionally with colleagues and provides support for smooth functioning of patient care team.
- Supports the department?s commitment to continuous improvement activities and principles through positive and respectful contribution in problem-solving and change processes
- May participate in research activities
PHYSICAL REQUIREMENTS:
The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations:
- Ability to walk or stand for long periods of time.
- Ability to communicate/exchange information; read, write, speak, and comprehend English.
- Functional visual acuity and hearing for accurate assessment and recording of patient observations and information in clinical records.
- Manual dexterity to perform required procedure activity proficiently and safely.
- Ability to move up to 35 pounds.
Ability to work in a sedentary position for long periods of time, fine motor coordination/dexterity, and may require reaching, bending, twisting, stooping, moving about.
Required Qualifications*
Nurse Practitioner
- Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
- Current specialty certification as an Acute Care NP or Adult-Gerontology Acute Care Nurse Practitioner
- Graduate of an accredited Nurse Practitioner program.
- Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
- Current DEA registration required.
- AHA Approved BLS certification.
- At least 2 years of clinical advanced practice provider employment experience.
- Current advanced practice provider employment.
Physician Assistant
- Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
- Certification by the National Commission on Certification for Physician Assistants (NCCPA).
- Current license to practice as a Physician Assistant in the State of Michigan.
- State of Michigan Controlled Substance License Required
- Current DEA registration required
- AHA approved BLS certification
- At least 2 years of clinical advanced practice provider employment experience.
- Current advanced practice provider employment.

Desired Qualifications*
- Previous experience as an APP in home care.
- APP experience in hospitalist medicine or emergency medicine.
- Experience with electronic medical records, specifically epic.
- Demonstrated proficiency with computer skills, Microsoft Word, e-mail, and smart phones.
- Ability to work non-traditional hours.
- Excellent communication skills, both verbal and written.

- Able to meet all credentialing requirements necessary to obtain required privileges of service.

- Ability to adhere to Michigan Medicine protocols for protecting patients and workforce members in the management of patients with confirmed COVID-19 and PUIs.
Work Schedule
Hours: 20
Shifts range from 10-12 hours in duration, and work hours include those that cover days and evenings. Holiday and weekend coverage is required.

Union Affiliation
This position is covered under the collective bargaining agreement between the U-M and the United Physician Assistants of Michigan Medicine (UPAMM), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
